Potential Threat to Patient Safety

A possibility of confusing two patients leading to misidentification has been identified,
from the simulation activity, as a potential safety threat. In this case, two nurses were discussing a
situation in which two patients with similar names and confusing birth dates were admitted in
rooms directly opposite each other on one floor. The two patients were B. Moore and B. R Moore
born on 08/11/05 and 11/08/05 respectively. One of the nurses is concerned that identifying them
with birthdates, room numbers or names is confusing and can lead to wrong identification.
However, the second nurse seems not to accept this position, claiming that the possibility of
confusing the two patients (wrong identification) is canceled by the fact that they are assigned to
different nurses at all times. However, this issue remains a safety threat because nurses resuming
their shifts can wrongly identify the patients due to confusion thus exposing the patients to more
health safety risks.

Implications of Not Addressing the Threat
